The 4 Ms in Action

Let’s think about how the 4 Ms might come into play in a made-up scenario. Imagine there’s a Dr. Yang who has a patient named Mr. Lewis.

What Matters

At their most recent office visit, Dr. Yang asks, “What’s most important to you these days?” Mr. Lewis smiles and shares that staying active in his garden and being able to travel with his family are his top priorities.

Mobility

Knowing what is most important to Mr. Lewis, Dr. Yang checks his balance and leg strength. She suggests a few simple exercises to help him improve his stability and encourages him to take regular walks. She even offers tips on making his home safer by removing slippery rugs and adding handrails.

Mentation

During each appointment, Dr. Yang asks gentle questions about Mr. Lewis’s memory, mood, and sleep, making sure he feels comfortable sharing. She listens for signs of forgetfulness or sadness, and she reassures him that it’s okay to talk about feeling down or anxious.

Medication

Dr. Yang carefully reviews all of Mr. Lewis’s prescriptions — along with the vitamins and pain relievers he buys over the counter. She checks for any possible drug interactions, explains which combinations to avoid, and ensures that everything is safe and effective for him.

By focusing on what matters, supporting his mobility, checking in on his mental health and cognition, and keeping his medicines safe, Dr. Yang is helping Mr. Lewis enjoy life on his terms. The 4 Ms work together to help keep him active, independent, and feeling his best.

Age-Friendly Health Care for Caregivers

If you’re providing care to an older person, here are some simple ways you can help your loved one experience age-friendly health care:

Go to doctor visits with them and ask questions.
Write down important information, like medicine schedules or treatment instructions.
Tell their health care team what matters most to your loved one.
Watch for changes in their mood, memory, or movement, and let the doctor know.
Help create a safe and comfortable home environment.
Do advance care planning with your loved one so their wishes can be followed.
Remember to take care of yourself, too; you are important!

But age-friendly health care is for you, too. It’s all about giving you the support you need to be successful and making daily life easier for everyone. If your loved one is an IBX Medicare Advantage health plan member, Carallel can help. This service is available to you at no cost and offers a wealth of assistance and resources to help you manage these sometimes difficult responsibilities.

Maximizing Quality of Life

Getting older brings new experiences and sometimes new challenges. Age-friendly health care’s goal is to make sure older people are living life as fully, healthily, and happily as possible, and that their families always have support, respect, and understanding.

Whether you’re navigating aging, or providing care to someone who is, remember that you’re not alone. Your health care team is here to listen, help, and work with you every step of the way. If you ever have questions or worries, reach out to them…because you matter, and your health matters, too.

Carallel, an independent company, provides information for general educational purposes and is not a substitute for legal, tax, investment, medical or other professional advice.

Independence Blue Cross offers PPO, HMO-POS, and HMO Medicare Advantage plans with a Medicare contract. Enrollment in Independence Blue Cross PPO, HMO-POS, and HMO Medicare Advantage plans depends on contract renewal.

Independence Blue Cross offers products through its subsidiaries Independence Hospital Indemnity Plan, Keystone Health Plan East and QCC Insurance Company — independent licensees of the Blue Cross and Blue Shield Association.
