6 Home Health Costs Medicare Usually Leaves to Families

Medicare covers many skilled home health services, but families are often surprised to learn they may still have to pay out of pocket for personal care, homemaker services, meal delivery, and other long-term support. PeopleImages/Shutterstock

Navigating the complexities of healthcare coverage, especially Medicare, can feel like a daunting task. It becomes even more stressful when you or a loved one needs assistance at home. Many families operate under the misconception that Medicare acts as a comprehensive safety net for all aging-related support, but the reality is much more limited.

While Medicare does provide vital coverage for skilled medical needs, it largely excludes the daily, non-medical assistance that many seniors rely on to live independently. There are some costs that are 100% passed on to the families. Here are six home health costs that many people are surprised to find out that Medicare doesn’t actually cover.

1. Around-the-Clock Care

Medicare is designed specifically for intermittent, short-term skilled care rather than the 24-hour monitoring that some patients require. If a patient needs constant, round-the-clock medical or personal assistance, Medicare will not foot the bill. This type of comprehensive care is typically considered a custodial arrangement rather than a medically necessary treatment plan. Families often find themselves needing to secure private pay arrangements or long-term care insurance to cover these extensive needs.

2. Personal Care and Custodial Help

Medicare generally pays for medical care intended to treat or monitor an illness or injury, while long-term assistance with everyday activities (often called custodial care) is usually not covered.  It is a common misunderstanding that Medicare pays for help with activities of daily living (ADLs), such as bathing, dressing, or using the bathroom. Medicare will only cover a home health aide for these tasks if you are already receiving skilled nursing or therapy services at the same time.

If personal care is the only service you require, you are generally responsible for paying the full cost out-of-pocket. This “custodial care” is excluded because the government does not view it as medically necessary skilled treatment. Without a concurrent medical need, families are left to either hire professional help privately or manage these daily responsibilities themselves.

Before Home Health Starts, Ask:

Which services will Medicare cover?Which services will I pay for myself?Will I receive an Advance Beneficiary Notice (ABN)?How often will caregivers visit?How long is the current plan of care?What happens when skilled care ends?

3. Homemaker and Household Services

General assistance with household chores—such as grocery shopping, house cleaning, or laundry—is rarely included in a Medicare-covered care plan. Unless these specific tasks are deemed part of a broader, medically necessary treatment plan, they fall into the category of “homemaker services”. Because Medicare is strictly focused on diagnosing or treating an illness or injury, general housework is not seen as a medical requirement. Consequently, families often have to rely on community resources, volunteer programs, or private services to keep the home environment maintained. Relying on Medicare for these tasks can lead to immediate denial of coverage, leaving families to bridge the gap independently.

4. Home-Delivered Meals

While nutrition is a critical component of recovery and health, Medicare does not provide coverage for meal preparation or home meal delivery services. Even if an individual is homebound and struggling to cook for themselves, this is considered a social or convenience service rather than a medical one.

Although Medicare doesn’t cover routine meal delivery, many communities offer nutrition programs through Area Agencies on Aging, Meals on Wheels, faith-based organizations, and local nonprofits. Eligibility varies, but these resources can help bridge an important gap for older adults recovering at home.

5. Long-Term Care Arrangements

Medicare’s home health benefit is fundamentally short-term, meaning it is not intended to support patients indefinitely. It is restricted to finite periods (usually 60-day certification cycles) that must be renewed by a doctor based on ongoing medical necessity. Because of this, it cannot be used as a substitute for long-term care plans, such as those provided in assisted living or extended home-based nursing.

Home health care is certified in episodes of care and must continue to meet Medicare’s medical necessity requirements. Coverage isn’t guaranteed indefinitely simply because someone prefers to remain at home. Once skilled care is no longer medically necessary, Medicare’s home health benefit may end even if assistance with daily living is still needed. If you require care that extends beyond the scope of “intermittent” skilled visits, you must seek alternative funding, such as Medicaid or private assets.

6. Deductibles, Copayments, and Coinsurance

While Medicare-covered home health services generally have no copayment, beneficiaries are typically responsible for 20% of the Medicare-approved amount for durable medical equipment, such as walkers, wheelchairs, or hospital beds, after meeting applicable Part B requirements. Many Medigap policies help cover some of these remaining costs.

Even when Medicare does cover a service, it rarely covers 100% of the cost for everything. Additionally, many patients encounter annual Part B deductibles before their insurance begins to pay its share. These out-of-pocket costs must be factored into your financial plan, as they are a routine reality of Medicare coverage.

Families facing ongoing caregiving needs may also want to explore Medicaid home- and community-based waiver programs (where available), veterans’ benefits for eligible former service members, long-term care insurance, state respite programs, and local nonprofit organizations that support aging in place. Availability varies by state and individual eligibility.

Medicare Does Cover Some Home Health Care

If eligibility requirements are met, Medicare may cover:

Intermittent skilled nursing carePhysical therapyOccupational therapySpeech-language pathologyMedical social servicesLimited home health aide services when you’re also receiving covered skilled careCertain medical suppliesDurable medical equipment (with applicable cost sharing)

Preparing Your Financial Future

Medicare coverage has its limits, and it is best for you to be aware of them before you need long-term care. By identifying which costs you will be responsible for, you can prioritize your savings and investigate state-based assistance programs like Medicaid waivers or veteran benefits. Remember that the goal of these services is to help you maintain independence, so researching all available community resources is just as important as understanding your insurance policy.
